Parents wishing to register a nursery-age or elementary-age child (for childcare or not) should complete the children’s registration form.

Youth registering for Annual Conference (under the age of 18) – whether visiting or attending as a youth Lay Member – should complete this form and also a liability release form. A digital signature by a parent/guardian is required.
